Insanity Mode Differences
Insanity Mode is an extra hard difficulty unlocked after beating the game for the first time. It features much harder enemies, new item locations, more expensive weapons, and much more.
Completing this mode is currently the game's ultimate challenge, so learn more about the differences below:
Stronger Enemies
First and foremost, Insanity mode features much stronger enemies in both the Grace and Leon sections of the game. Grace in particular will die within two hits to most enemies, and enemies also have more health forcing you to use more supplies.
Insanity mode also features more enemies overall, most noticeable during the Leon sections.
For the most part, it is recommended to avoid as many enemies as possible in this mode. The game does offer some reprieve, in that Leon begins earning points for his kills from the very start of the game, and enemies give more points in general on insanity.
New Item Locations
You will quickly notice that some items have had their locations moved, most notably the Antique Coins. There are now Antique Coins hidden in new places throughout the game (including in the intro segments), and certain bosses now drop Antique Coins instead of their usual loot. Almost every Key Item required for progression has stayed the same, with a few small exceptions:
- B934 Handgun is now found in the East Wing Waiting Room.
- New Empty Injector found in the West Wing Kitchen.
- Unicorn Trinket Box is now in a different spot in the same room (Chairman’s Office).
- Pantry Key is no longer dropped by the Chef Zombie, now found in the East Wing Waiting Room closet.
- Some Coin Locker Rewards have changed: Handgun Ammo and R.I.P. Knife replace the Steroid and Stabilizer.
New Safe Combinations

More Expensive Weapon Upgrades
Weapon upgrades also cost more on Insanity. Most are roughly double the cost of normal mode, though there are some outliers that are exponentially higher. Learn more in our Weapons guide.
