This required several breaking changes to be taken into account.
I reached a point when I can successfully populate the database and
start a development server. Next steps is writing tests to ensure that I
did not make a mistake.

This is a integration test. Turns out, the way the middlewares are
currently written makes it impossible to test them in isolation.
Therefore I'm following the best practice and run tests with supertest
against a running database.
