I suggest more natural language, less abbreviations and punction:

That seems good and clear enough. My only thought is whether it can be shortened so the key points stand out more easily, and I think active language is better than passive here. I think this reads better and more directly:

When informing someone about their inappropriate behavior, communicate privately through shared accounts (via mail or matrix) if possible. Endeavor to clearly explain the situation and assist the person in successfully resolving the issue.

That uses imperative mode instead of "moderators should" language. If you really prefer the other, it's easy to swap that in instead.

I used "moderators should" to make it more consistent with the rest and because also non moderators will read it andd then communicate privately sounds incorrect (for me) 🤔

"communicate privately" is normal natural English, though there are alternative ways to say the same thing. On "moderators should", that's just various trade-offs and relates to who is reading. I agree that consistency across the document is best.
