When comparing the confirmed lists of countries (LOGic and Lotw) I found OK3 was the extra DXCC entity in LOGic and was listed as "SLOVAK REPUBLIC" OM OM,OK3 in the Prefix Table. Instead of "CZECH REPUBLIC" OK OL I deleted the "OK3" and did a reindex & pack.
