I had an interesting experience for my first submission, in late 2006, where the DXCC desk questioned my submission of a KH7 card for Hawaii. It said Hawaii on the card so that's what my application submitted to field checker KE9ET listed. They thought it might possibly be Kure. 
We got it straightened out with a couple of emails and faxing the card in. Yes, it was Hawaii. So someone was thinking a little too hard bur the intention was good.
