Fill in the form using Standard PDF or PDF Form.
Please note that you have to use your full name as CN basically.

Send it to KEK Computing Research Center Administration Office by FAX (+81-29-864-4402) or usual mail with the copy of your photo ID. Please be careful for clearness of the photo.
At least you many need to enlarge it and to use "photo" mode when you fax it.

Please specify your preferred method to return the necessary information from us otherwise we mail it (regular mail).
