David, I switched to DCA net from DirecTV, main reason: multiple static IP addresses. Here's what was on my first invoice, note that it includes the purchase of a new ADSL "modem": 1 each Verizon self install kit 219ドル. 1 each Verizon activation fee 60. 1 each Federal Universal Service Fee 4.46 1 month Verizon 768/128 ADSL Connection 49.95 1 month Federal Universal Service Fee 2.91 Subtotal 336ドル.32 Sales Tax 15.31 Amount Paid 351ドル.65 So the sales tax is approx. 7% (6%-PA, 1% Phila.), assuming that only the "self install kit" was subject to it, the "federal universal service fee" (FUSF) is approx. 5.8% using the 2.91/49.95 ratio. I'm not sure what the one-time 4ドル.46 fee is based upon. Judging by this invoice, I expect that the FUFS will be added to my 49ドル.95 bill each month.
