I believe that all you need is available from the DXLab DXKeeper
application, freeware.

Subject: [Yaesu] QSL Card's
I have been hunting high and low for software that can do a few simple
things:
1. Allow me to create and print QSL cards.
2. Import an .adif log for the QSL info on the cards.
3. Most importantly, the ability to retrieve mailing addresses from the
Radio Amateur Callbook CD from the ARRL and put them on the opposite side of
the card.
Mainly I have been hunting for software that takes care of these items, but
haven't found any. I have now been using QSL Maker to create the cards,
although I like using that software it does have the drawback of not being
able to do the printing of the address side.
I have tried just about every logging software under the sun as well. So,
anybody have a method or software for making a complete QSL card without any
handwriting of information or addresses? Or, even just a program to
retrieve and print the mailing addresses on the back of the cards made with
QSL Maker. I make a lot of cards.
