Hope this is of use:-
I followed the instructions described on pages 9 and 10 of the 425 DX 
News magazine June 2007 - here is the link

BUT - this info may not be good now - strongly suggest you e-mail Vlad 
and ask him if you can still pay via PayPal.
On his return from last African trip he sent me a form (his own travel 
company) which I filled in and faxed to his office with amounts etc.
I also downloaded all contacts from my log and put them on a 
spreadsheet in chronological order, then e-mailed them to Vlad at the 
same time (with return address) - AND included reference to the fax I 
had sent a minute earlier.
This was acknowledged and he promised to send cards ASAP - had to wait 
a while for receipt (April 2008!) but he did make over 300 k QSOs so 
that was understandable. He should be fairly quick now that the 
backlog will have been squared away.
I wish you luck - would urge you first to e-mail Vlad for a MAYBE more 
up to date method and to get the ball rolling, so to speak.
BTW: I sent him 25 USdollars for approx 85 band slots - well worth it 
bearing in mind this bypassed (and saved on) postage costs to him (and 
the attendant worry that QSLs would be received at his end!) AND he 
paid the postage for cards to me.
