I am your local DXCC field checker. Here is the scoop:
1) The cards are first sorted by band and then within each band grouped
by mode. Post it notes are a good way to indicate if a Multiple QSO card
is only to have one QSO counted (making that card a single QSO card).
Multiple QSO cards are grouped at the end of the pile, with no special
order. If there are QSOs that you don't want counted (pay for) you should
use something like a Post-It note on the cards to indicate which ones to
count. A note like Please count only 20mtr and 30mtr QSOs is sufficient.
2)Just draw a single straight line thru the listing on the form. You can
white it out, but I wouldn't write over it.
3)& 4) The form listing the QSOs can have blank lines in it. The form
with the listing of QSOs is only needed for hamfest and other field
checking. It is the form with the listed QSOs that goes to ARRL. You do
not have to use the form for listing the QSOs. You can use a reasonable
facsimile of the form made by your computer. The columns need to be in
the same order and your call or calls should be on the print out. Some
log award tracking programs will automatically produce an acceptable print
out. I believe that you are required to have the "application" part of
the ARRL forms with any application. This is the part with which award
you are applying for , your address, calls used, etc. This form also
shows how you are paying for the submission. With a field or hamfest
checked application (no cards to ARRL) the fee is easily computed and a
check or credit card can be used as payment.
If I am checking the cards, I am not as strict about card order unles you
are bringing me a 5BDXCC type package. HQ is strict about the card order
and the order on the print out (band-mode then multi-Q cards).
5) Unfortunately, cards submitted before 1985 ish were not recorded. Also
if you submitted cards for a 5BDXCC around that time frame there was no
recording of the band and mode. In some cases, the only thing remaining
was which countries you have worked. There is a slight discount if you
are applying for several single band awards. Essentially HQ allows you
some (payment) credit for previous submissions like with a 5BDXCC. But
there is some sticker shock in trying to get up to the challenge award
level if your starting for an old DXCC award.
There are two other items: First, cards that date back more than 10 years
must be either checked at a hamfest/convention (with DXCC staff present)
or checked at HQ. Also 160m cards need to checked by HQ staff either at a
hamfest or at HQ. That means prying the cardboard out of your hands and
entrusting those priceless cards with UPS, USPS, or Fedex. You could of
course carry them there for an in person checking session ( I think that
you need to schedule a drop by of more than 120 cards). So there is some
scare factor of parting with those cards and the expense. Secondly, for
field checked cards, you need to supply an envelope with postage to the
field checker. The field checker needs to mail this within 48 hrs. I
suggest if you submit cards to HQ, that you use a credit card to pay for
the awards and return postage. If you send too much you'll have a credit
and if you send too little you'll get a bill (before you get an award).

I have some questions on how to fill out the DXCC application.
1. My understanding of the FAQ is that the stations and entities within
a band-mode category can be in any order - is that correct?
2. How do I delete a line if there is an error I can't fix?
3. Am I permitted to have blank lines separating a band-mode in case I
miss something in a band-mode?
4. Can I type my own form that looks as close to the ARRL form as
possible? It's much easier to sit down at a computer and type in the
information rather than fill in a form by hand, especially with my
current handwriting <g>
5. My last submission was in 1950 something. My understanding is that
I would have to submit current cards in order to qualify for band-mode
awards. That is, submit new cards where possible.
