I'll add to that. Paul and I are both tired of people getting the date,
time, etc. wrong on their cards, let alone my silly call (funny, they
seem to find the mailing address ok). I use DX4WIN to log. Believe me,
it shows the correct date and time, expecially on vacation where we're
working lots of contacts. 
Paul paper logs except for contests. It's really hard for him to find a
match on some cards but he tries really hard, like I do electronically.
If the cards are sent to us with the call off by a letter, we still
confirm them if we can find them in the log with the right mode and
band. That's the important parts to us. Otherwise we have to tell the
sender "not in log."
Once in a while we'll log a call wrong ourselves and find out when a
request comes in.
Other ops probably have different standards for confirming QSOs.
