If we leave out that from a sentence like this: The truth is that they don't even realize their mistakes.,
should we insert a comma after The truth is (The truth is, they don't even realize their mistakes.)?
Under normal rules of punctuation, there shouldn't be a comma between the verb and the complement, so we can probably defend the position that there should be no comma.

However, we sometimes use punctuation to reflect speech style, and there can of course be a pause after is. So if this is the case (for example in short story where someone is quoted), you could insert the comma.
